- The policyholder must be a Hong Kong resident aged eighteen or above.
- Your pet should not have received or required treatment for accidents or illnesses in the past 90 days, excluding routine check-ups or preventive vaccinations.
- Except for neutering, your pet should not have undergone any surgical treatments.
- Your pet should not have shown any violent tendencies or attacked/bitten any person or animal in the past 5 years.
- Your pet should not be used for commercial purposes.
- Your pet should not have any physical defects or disabilities.
- When applying for this insurance plan, you need to provide your pet's microchip number*.
*Under Hong Kong regulations, all dogs or imported cats must have a microchip implanted. If your cat is not an imported breed and does not have a microchip, we recommend implanting one. While it does not affect eligibility for this insurance plan, it can help in quickly locating your cat if it goes missing, ensuring a safe return home.
Age limit for your pet:
Cats or dogs aged 6 months to 8 years (last birthday) are eligible for coverage.

• An incomplete insurance order does not constitute a policy. bolttech insurance (Hong Kong) Limited will issue the formal policy document only upon receipt of the required information and acceptance of the insurance application.
• If within 30 days from the date of purchasing the insurance order, you have not successfully completed and submitted the insurance details, the funds
previously used for the incomplete insurance order will be converted to Mall Dollars and deposited into your HKTVmall account. Any funds deducted under any discount offer will not be considered as part of the total payment value for Mall Dollars calculation.

Before completing this insurance order, I hereby declare and agree:
- I have read, fully understood, and accepted bolttech insurance (Hong Kong) Limited's privacy statement, product brochure, and product policy terms and conditions, and agree to be bound by them.
- The information and details provided in the insurance application are accurate, truthful, and complete to the best of my knowledge and belief. I have not concealed any important information, and I agree that the content and statements of the insurance application will form the basis of the insurance contract between bolttech insurance (Hong Kong) Limited and me. I confirm that failure to provide true and accurate information or notify bolttech insurance (Hong Kong) Limited of any important information related to the insurance application may result in the rejection of the insurance application or render the policy void.
- Coverage will only take effect after the insurance application is completed, accepted by bolttech insurance (Hong Kong) Limited, and the applicable premium has been paid.
- (If applicable) I have been authorized by the insured to provide all necessary information required for the insurance application, negotiate with bolttech insurance (Hong Kong) Limited on matters related to the insurance application, and receive or request information related to the insured. I confirm that the insured has been clearly notified and agreed that their personal information will be transferred to bolttech insurance (Hong Kong) Limited for processing the relevant insurance application and for the purposes stated in bolttech insurance (Hong Kong) Limited's privacy statement, and has been informed of their rights under the Personal Data (Privacy) Ordinance. bolttech insurance (Hong Kong) Limited will not be liable if the insured is not informed accordingly.
- This policy is applied for within the Hong Kong Special Administrative Region.
- I understand and agree that bolttech insurance (Hong Kong) Limited will pay commissions to authorized insurance agents responsible for arranging the policy on my behalf during the policy period.
- I also understand that bolttech insurance (Hong Kong) Limited must obtain the above consent from the applicant to process their insurance application.
